Before your appointment:

Please print, fill out and bring the required forms with you to the clinic. The forms are located under the BOOK ONLINE tab. Please select the ones appropriate to the type of appointment you have booked.

We require a 24-hour notice for all cancellations. If a 24-hour notice is not given, new clients will be charged for 50% of the cost of their appointment and established patients will be charged a $185 fee.

We ask for a valid credit card to hold all new patient appointments.​

All purchased products and completed treatments are non-refundable.
